2. Enter your personal information exactly as it appears in your patient record:
• First and last name (no nicknames)
• Email address
• Date of birth
• Gender
• Last 4 digits of your Social Security Number
• User ID (8-16 characters, letters and numbers only)
• Password (8-20 characters, at least one letter and one number)
• Select and answer three challenge questions for security.
3. Verify your identity using one of the following:
• Health insurance number (from your insurance card)
• Billing account number (from your Mercy statement)
• Access code (provided by your provider if needed).
4. Review and accept the terms and conditions.
5. Submit your registration. If all information matches your patient record, your account will be activated immediately.
